Last pic was a close-up of lid. This is the whole thing, complete with name tag.
So, I explained most of it in the last picture, but not the tag. I made the tag by gluing printer paper onto a square of red cloth (the elastic bookcover), then gluing the edges of the excess cloth over onto the paper. For security and aesthetics, I sewed the paper into the cloth on all sides with white thread, then sewed the tag onto the box. Want to know what's inside the box?
